Efectividad del uso del Bionator en la maloclusión clase II división 1: reporte de Caso / Effectiveness of the Bionator appliace in class II malocclusion division 1: case report

El Bionator de Balters Estándar es uno de los aparatos funcionales más utilizados para el tratamiento de la maloclusión clase II división 1, su efecto consiste en potenciar el crecimiento y reubicar anteriormente la mandíbula. Existen muchos factores para el éxito del tratamiento, algunos de ellos son: el momento de inicio de tratamiento y no menos importante, la colaboración del paciente. El uso de este aparato queda confinado a las etapas de pico de crecimiento pre-puberal, como fase inicial del tratamiento ortodóncico con aparatos fijos. El presente artículo presenta un reporte de caso de una niña de 9 años en estadío 2 de crecimiento según el Método de Maduración de Vértebras Cervicales, con presencia maloclusión clase II división 1 que fue tratada con Bionator de Balters Estándar. En el período de 1 año de tratamiento se observó un aumento considerable de la distancia Co-Gn (de 116 mm a 123.5 mm). Cambios significativos a largo plazo y sobretodo en el incremento del crecimiento mandibular, pueden ser alcanzados mediante el tratamiento con Bionator cuando el tratamiento funcional incluye la etapa prepuberal.

Standard BaltersÆ Bionator is one of the most popular functional appliances for the treatment of class II malocclusion division 1. The effects of the applance consists in the promotion of growth and the relocation of the jaw anteriorly. There are many factors for the successful treatment, like: the time of the start odf treatment and the patientÆscollaboration. The use of this appliance is confined to the pre-puberal peak growth, as an initial phase of the orthodontic treatment with fixed appliances. This article presents a case report of a 9 year old girl in cervical stage 2 according to the cervical vertebral maturation method, with the presence of class II malocclusion division 1, that was treated with Standard BaltersÆ Bionator appliance. In the period of 1 year of treatment we observed a significant increase of the distance Co-Gn (from 116 mm to 123.5 mm). Significant long term changes and increase of the mandibular growth can be reached by the treatment with Bionator appliance when the functional treatment includes the pre-puberal stage.

Es de considerar que la maloclusión Pseudo Clase III puede desarrollase en los niños como resultado de la presencia de contactos oclusales prematuros que provocan un desplazamiento anterior de la mandíbula. Para establecer un tratamiento adecuado de esta maloclusión es importante hacer el diagnóstico diferencial correcto. Cabe mencionar que varios aparatos se han diseñado para el tratamiento precoz de una Pseudo Clase III; aquí presentamos tres casos tratados con diferentes aparatologías teniendo como resultado final el éxito del tratamiento.

The Pseudo Class III malocclusion can develop in children as a result of the presence of premature occlusal contacts, causing the move up the jaw. To establish an adequate treatment of the malocclusion is important to make the correct diagnosis. It is worth mentioning that several devices were designed for early treatment of pseudo Class III malocclusion, here we present three cases treated with different appliances resulting finally in the treatment successful.

El paciente infantil desde el punto de vista de la ortodoncia-ortopedia facial / The child patient from an orthodontic facial orthopedic point of view

En el presente artículo pretendemos de una forma sencilla y clara visualizar la importancia que existe de integrar dos especialidades muy afines, como son la Ortodoncia-Ortopedia Facial y la Odontopediatría; teniendo la obligación ambas de acompañar durante todo el crecimiento y desarrollo a nuestro paciente niño. Explicaremos lo importante de esta relación desde el diagnóstico, hasta llegar finalmente a determinar el momento ideal y que formas de tratamiento preventivo e interceptivo (dependiendo el caso)podemos llevar a cabo para lograr el éxito. Asimismo indicaremos todos los elementos necesarios a considerar para realizar una correcta evaluación y por consiguiente obtener un diagnóstico ortodóntico temprano; conocer que debemos tratar, y así determinar correctamente en que momento y que metodología a seguir.

In this article we try to point out in a clear and simple manner the importance of integrating two related specialties Facial Orthodontic-Orthopedic and Paediatric Dentistry, having both a common goal which is to accompany the growth and development of a child. We are going to explain the importance of this connection from the diagnosis to finally determine the ideal moment and the forms of preventive and interceptive treatment (according to each case) we can carry out to succeed. Also we will point out all the elements we need to consider to do a good evaluation so as to get an early orthodontic diagnosis and thus accurately determine the timing, the reason and the methodology to follow.
